Are you missing an online performance document for a team member? All MaPS and most Support Staff automatically receive an online review document every year. Managers can request performance documents for employees that don’t already have them.
Once submitted, a member of our Talent Development team will respond within five business days.

Please note: Given that we are approaching the end of the current support staff annual review period (Mar 31st 2020), we will accept requests for online document creation for the current cycle until March 16th. After this date, for any support staff that are not set up with online documents, annual reviews can be completed on paper. Access the Paper Self- Assessment (employees). Access the Paper Manager Assessment (managers). Once signed, managers submit the completed reviews to their HR Advisor to be placed in the employee file.
